Prepare for Your Visit

Your Comprehensive Evaluation

Your comprehensive consultation and evaluation is designed to discover key strengths and weaknesses in individuals struggling with learning, developmental, and behavioral challenges.

You will receive an expert assessment and recommendation to map treatment and action plans for you.

We provide a comfortable environment for parents and family members to wait during the extended assessments with the patients. Please plan accordingly.

Arrival Protocol

Please arrive at the office at least 30 minutes before your appointment time. If you will be delayed please call our office as soon as you can. You can find directions to our office on this page.

What to Bring

Please bring any medications the patient is currently taking. Also bring any relevant documentation from school, IEP, counselors, or medical records, recent imaging or x-rays.

You can download and fill out the New Patient Packet and Signed Forms provided below. You may print and fill these out at home and bring to your appointment to save time.

Cancellation Policy

If you need to cancel or reschedule your appointment please do so at least 24 business hours before your scheduled appointment. You may be subject to cancellation fees.

For first time appointments 48 business hours’ notice is required or the deposit will be retained and non-transferable.

Your Health Insurance

We are not contracted with any insurance carriers; therefore we do not accept payment from insurance carriers.

Our goal is to offer premium care and service specific to each patient’s needs without having a third party payer dictate a patient’s diagnosis or treatment plan.

As a courtesy to our patients we submit an insurance claim for direct reimbursement for the services rendered at the appointment. Click Here to see a full description of our Financial Policies.

Payment Support Options

Financial burdens related to the cost of our comprehensive evaluations frequently discourage patients from pursuing appropriate measures. The Payment Support offered by the DDC is an additional resource for your consideration in overcoming any financial limitations you might be facing while making your decision to get the appropriate evaluation.
